I authorize regularly scheduled charges to the credit card provided as outlined in the service agreement. I agree that no prior notification will be provided unless the transaction amount is changed. If the transaction amount is changed, you will receive 30 days notice prior to the payment being collected. A receipt for each payment will be provided and the charge will appear on your credit card statement.
